Risk Acknowledgment Form

1. Acknowledgment of Inherent Risks

I acknowledge that stand-up paddleboarding (SUP) is a water activity involving inherent risks that may result in serious injury or, in rare cases, death.

These risks include, but are not limited to:

Falls and immersion in water
Loss of balance or inability to get back onto the board
Physical effort and fatigue
Hypothermia or heat stress
Currents, waves, and changing water conditions
Rapid weather changes
Marine traffic or presence of other vessels
Distance from shore or emergency services

I understand that this list is not exhaustive and that other unforeseen hazards may arise.

2. Natural Environment and Access to Emergency Services

I understand that this activity takes place in a natural aquatic environment that may be far from medical or emergency services.

I understand that response times may be longer than in an urban environment.

3. Voluntary Participation

I voluntarily participate in this activity and accept the associated risks.

I understand that my safety also depends on my active participation, judgment, and respect for the instructions provided.

4. Physical Condition and Health Status

I declare that I am in good physical condition and fit to participate in an outdoor water activity.

I confirm that I do not suffer from any medical condition, injury, physical or psychological limitation, pregnancy, severe allergy, or any other condition that could compromise my safety or that of the group.

If I have any particular condition, I agree to inform Aquaventure Charlevoix before the activity begins.

I understand that any medical condition or physical limitation may increase the risks associated with the activity and I voluntarily accept these additional risks.

5. Ability to Swim

I declare that I and all participants I register are able to swim and are comfortable in the water, including deep water.

6. Participant Age and Supervision

I declare that all participants are 12 years of age or older.

Any minor must be accompanied by a responsible adult according to the ratio established by Aquaventure Charlevoix.

7. Compliance with Instructions and Supervision

I understand that all measures implemented by Aquaventure Charlevoix aim to ensure participant safety.

I agree to follow all safety instructions provided by staff at all times.

I understand that failure to follow these instructions may result in immediate termination of my participation without refund.

8. Mandatory Safety Equipment

I agree to wear the provided personal flotation device (PFD) at all times during the activity.

I also agree to keep the leash attached to the paddleboard.

I understand that failure to comply may result in termination of my participation without refund.

9. Board rental and Intro/Rental – Technical Ability

As part of a board rental session or an intro/rental activity, I understand that I must be able to get back onto the board independently, without assistance or contact with the bottom, and demonstrate a basic level of navigation ability, including maintaining a stable direction.

If this ability is not demonstrated, the activity may be restricted in terms of navigation area or discontinued for safety reasons, without refund.

10. Alcohol, Drugs, and Substances

I understand that participation under the influence of alcohol, drugs, or any substance impairing my abilities is prohibited.

I understand that participation may be refused or terminated without refund if I arrive under the influence.

11. Weather Conditions and Activity Changes

I understand that the activity may be modified, shortened, rescheduled, or cancelled if weather or water conditions become unsafe, including strong winds, large waves, storms, thunder, or other risky situations.

I understand that these decisions are based on the professional judgment of Aquaventure Charlevoix staff.

12. Borrowed Equipment – Responsibility

I acknowledge that the equipment provided is in good condition.

I agree to use the equipment properly and return it in similar condition.

I understand that I may be held responsible for damage, loss, or breakage caused by negligence, misuse, or intentional acts.

13. Schedule and Late Arrival

I understand that I must arrive at least 15 minutes before the activity.

I understand that late arrival may result in a shortened activity without refund.

I understand that late return of equipment may result in additional fees.

14. Personal Belongings

I understand that I am responsible for my personal belongings, including phone, glasses, keys, clothing, and other items.

Aquaventure Charlevoix cannot be held responsible for loss or damage to personal belongings.

15. Right to Refuse or Remove Participants

I understand that Aquaventure Charlevoix reserves the right to refuse or terminate my participation if my behavior, physical condition, or technical level represents a risk to myself or the group.

16. Authorization for First Aid and Emergency Medical Care

In case of accident or emergency, I authorize Aquaventure Charlevoix staff to provide necessary first aid.

I also authorize Aquaventure Charlevoix to request medical assistance or evacuation services if deemed necessary, including ambulance or other emergency transport.

I understand that associated costs may be my responsibility.

17. Acceptance of Terms and Conditions

I confirm that I have read the Terms & Conditions of Aquaventure Charlevoix and acknowledge that they form part of my participation.

18. Final Declaration and Consent

I confirm that I have read, understood, and accepted all conditions of this form.

I declare that all information provided is accurate and complete.

I understand that false statements may result in immediate exclusion from the activity without refund and may limit my legal recourse.
